Did you know that you can order your student transcript from St. Thomas More School? The request form is located below. Click on the 'PDF' icon to download the request form. Just mail the form to the Archdiocese Archives & Records Center (711 West Monroe Street, Chicago, IL 60661) with $10 (cash, cashier's check, or money order), and you will receive your record in the mail! Please note: when sending the request you must also include a copy of a picture ID for verification.
